Every practice exam uses a small number of credits, based only on how many questions you choose.
Credits are added to your account by your administrator. This is exactly what each exam size uses:

Only your administrator can add credits to your account. New accounts start with zero credits until activated.
This depends on your administrator's policy. Ask them directly if you're unsure how long your credits are valid for.
You'll see a low-balance warning on the setup screen before you run out. Contact support or your administrator to top up.
Credits stay in your account until you use them — they are not reset automatically.
